چکیده

Failure of a primary operation to control renal hyperparathyroidism is usually due to inadequate resection of hyperplastic tissue or previous undetected ectopic parathyroid gland. We describe a patient with recurrent renal hyperparathyroidism, who presented with a subcutaneous neck nodule. A 49-year-old woman had a history of systemic lupus erythematosus, papillary thyroid carcinoma, and end-stage renal disease with renal hyperparathyroidism, for which she subsequently received percutaneous ethanol infusion therapy to bilateral hyperplastic parathyroid lesions, a subtotal thyroidectomy and parathyroidectomy. Despite aggressive treatment with phosphate binders and vitamin D3 supplements, the levels of intact parathyroid hormone, calcium and phosphorus kept elevating during the follow-up period. Thyroid sonography revealed neither thyroid nor parathyroid tissue in the thyroid bed. Physical examination showed a palpable subcutaneous nodule at the right neck, which was hyperechoic and heterogeneous on soft tissue sonography. Technetium (Tc) sestamibi scan showed a focal hot spot at the same site. She received complete surgical resection of the neck nodule. Histopathological analysis showed parathyroid hyperplasia. Two years after removal of the nodule, the patient was well. In conclusion, to avoid recurrent hyperparathyroidism, a meticulous surgical technique is vital to successful parathyroid surgery. On the other hand, alcohol injection is not recommended for renal hyperparathyroidism if the patient can tolerate operation, because alcohol injection can cause fibrosis which makes further operation difficult in processing. We should also keep subcutaneous parathyroid lesions in mind when we perform neck re-exploration for recurrent hyperparathyroidism. (J Intern Med Taiwan 2011; 22: 206-211)

Intensification of Anemia by Secondary Hyperparathyroidism in Hemodialysis Patients

The excessive amounts of parathyroid hormone in secondary hyperparathyroidism (SHPTH) is suggested to interfere with normal erythropoiesis.  In SHPTH, during chronic renal failure, due to the impairment of erythropoietin synthesis, this effect is more pronounced.  In the present study the role of secondary hyperparathyroidism in the severity of anemia was evaluated in hemodialysis patients (n=3...

Huge Parathyroid Adenoma with Dysphagia Presentation; A Case Report from Southern Iran

AbstractParathyroid adenoma is a benign tumor of the parathyroid glands. The cause of most parathyroid adenomas is unknown. Parathyroid adenoma increases the secretion of parathyroid hormone and results in primary hyperparathyroidism. High amounts of parathyroid hormone in the blood cause the imbalance of calcium, which leads to various complications such as kidney stones, depression, lethargy,...

Significance of MIBI scintigraphy in a patient with hydatid cyst and parathyroid adenoma

A 17-year-old boy with hyperparathyroidism exhibited a large cyst in the left hemi-thorax on his chest X-ray. The cyst was identified as a hydatid cyst. He was referred for biphasic methoxy isobutyl isonitrile (MIBI) parathyroid scan. A discrete ectopic parathyroid was detected in the left upper mediastinum in addition to accumulation of the MIBI tracer in the cyst wall. Using radio-guided occu...

Sagliker Syndrome in a Patient with Secondary Hyperparathyroidism and Chronic Renal Insufficiency: A Case Report

Sagliker syndrome is a rare form of renal osteodystrophy resulted from untreated secondary hyperparathyroidism. It is described by severe skeletal deformities, high level of PTH in patients with chronic renal failure, and deformed face. This paper reports a 44-year-old male patient with the mentioned characteristics. In addition to the unique clinical features, high levels of ALP and PTH hormon...
